- 1st drop down box, select the kind of leave being requested
- 2nd drop down box, select the appropriate leave type

Sabbatical

A sabbatical results summary report must be submitted for all previous sabbaticals before a new sabbatical leave can be requested. All new sabbatical requests require a sabbatical request report. Once submitted, this form will route to your Academic Personnel representative and Chairs/Directors for their review/approval. Graduate Student Eligibility Checklist
In order to be employed or receive fellowship funds, graduate students should be in good standing and enrolled in 12 units, among other eligibility requirements. Please complete this prior to the start of each quarter in order to verify your eligibility for employment and/or fellowships. Turn the form in to the graduate program coordinator.
- Timesheet Information for Students and Staff
All student and staff positions require time a timesheet either monthly or biweekly. Submit your timesheet on time in order to be paid on time.
